Immersed in a thick wood, the small church was built by the Contelori family on the site of an older oratory.
The outside is typical of devotional churches, with two small windows on either side of the simple terracotta portal.
The interior, with a single nave, has an interesting fresco by Bartolomeo Barbiani, dated 1638, representing Our Lady with Child in glory between St. Biagio and St. Carlo Borromeo.
Between the two saints one can admire a view of the suggestive landscape with the village of Cesi.
In the two niches at the sides of the painting, St. Anthony the Abbot is depicted on the left and St. Joseph on the right.
