The church, built in 1675 on an ancient hermitage immersed in the wood of St.Onofrio, stands in a dominant position overlooking the village of Cesi and the Terni valley.
After an initial restoration in 1906, as recalled by an inscription on the right of the altar, in 2014 it was in a state of complete neglect until some interventions led to the arrangement of the path leading up to the site, of the small square adjacent to the church and the interior. Part of the adjacent building still needs to be restored, which at present is unsafe and not accessible.
The exterior is in the classic votive building style.
The gabled facade is surmounted by a tympanum with cubed decorations, at the centre a brick portal flanked by two small windows opens up, a larger one is placed above to give light to the interior. The bell tower is in brick, ribbed with a double arch and without bells.
The interior features only a single nave, on the raised main altar one can admire what remains of the polychrome stuccos and two angels. Here the statue of St. Onofrio, patron saint of the village was located.
